order to utilize services provided by Coloradohealthpsychology.com you must
complete an intake form where certain fields are required prior to performing
services. Your name, address, telephone, email, and a brief statement of your
problems will be required information during the intake process. Your personal
information will only be used as necessary to contact you if you are a
registered customer with pending sessions or post-session issues. The
information required is necessary to ensure accuracy of billing and your email
address will only be used for customer contact when a session has been
scheduled. Email addresses may be used for Coloradohealthpsychology.com
marketing and mailing lists, but only if the user has elected such services. We
do not sell, rent or release any information gathered on our site to third
parties. All information gathered will be used only for internal marketing
analysis and to improve our products, services and websites. You will have a
choice to opt-out of solicited emails during the registration/intake process.

Some clients may wish to remain anonymous, that is, have treatment without the psychologist knowing their name. Such requests are problematic and if a psychologist agrees to such an arrangement, the agreement can only be tentative and conditional. Therapists have a right, and in some situations, an obligation to know who their clients are. One reason for this is the issue of "dual relationships," the other is compliance with the law regarding risks to or presented by clients. For this reason we require personally identifiable information prior to issuance of services.
To maintain anonymity, you may elect to enter false information in the required at your own discretion. Although, Colorado Health Psychology, LLC, Coloradohealthpsychology.com and your psychologist cannot be held liable under such circumstances.
Our information systems keep a record of the secure chat between visitor and clinician. The content of the online consultation is securely retained for legal and administrative purposes only and will not be used for any other purpose other than a recorded record of conversations.

Tracking Information

collect information about our visitors for hit tracking and to help diagnose

private between you and your therapist only. Your psychologist is located in a
secure, private suite where the ability to overhear telephone, voice chat, or
in-office conversations has been restricted. Users are automatically logged off
of their accounts after a timeout period has been reached.
WE USE APPROPRIATE SECURITY SAFEGUARDS.
Confidentiality
United
States law requires that any suspicion of risk of child or elder abuse be
reported to welfare authorities or law enforcement authorities. If a
professional believes that a child or elder is at risk of being abused, has
already been abused, or that someone who previously abused children or elders is
still a threat, they will often make an effort to report it to social services
agencies and/or the police.
Most
United States courts have the power to subpoena our records and a professional's
records if you are involved in a court case in which your relationship with
Colorado Health Psychology, LLC(Coloradohealthpsychology.com), or the
professional is somehow relevant. Although this may be rare, if you have any
questions, it is better to contact your lawyer prior to disclosing any

If someone's life is in danger, the
